The Wandering IOL: A Rare Case of Pseudophacocele

This case was operated a few months ago and the patient presented to our hospital during a camp with complaints of decreased vision for the past two months.

On slit-lamp examination, the findings were truly jaw-dropping.

This is a rare and striking entity known as Pseudophacocele.


Pseudophacocele is characterized by the migration of the intraocular lens (IOL) into the subconjunctival space. The most common cause is trauma, although other mechanisms may also contribute.


Management involves performing a conjunctival peritomy, carefully explanting the IOL, and suturing the conjunctiva. The decision for secondary IOL implantation depends on the ocular and posterior segment status. However, since trauma is the predominant etiology, associated posterior segment damage often influences the final visual rehabilitation plan.
